PARENT/GUARDIAN PERMISSION FORM

Dover Public Library and the City of Dover require permission for anyone under the age of 18 to participate in any volunteer activity. Youth volunteers must have a current Parent/Guardian Permission Form on file at Dover Public Library in order to volunteer.

All volunteers, including youth, are responsible for their own transportation to and from Dover Public Library activities.

Youth and Parent(s)/Guardian(s): Dover Public Library appreciates your willingness to volunteer your services to assist the Library, its patrons and the community. In signing this form as a Volunteer, you are acknowledging that your services for the Library are gratuitous and are intended as a contribution by you for public service for the Library, its patrons, and the community, and as such, that you will be entitled to no compensation or any fringe benefits or other employment rights applicable to the employees of the Library. It is expressly understood that you are not an employee or agent of the Library and that for any reason whatsoever either you or the Library may terminate the volunteer arrangement. While on Library premises you will agree to abide by all of the rules of conduct governing the staff and employees of the Library in performing your services.  As a Volunteer, you hereby acknowledge that you will perform your services in good faith and to the best of your ability under the general guidelines provided to you.

As the parent or legal guardian of the minor signing this form, this applicant has my permission to volunteer at the Dover Public Library. I grant permission for my child to access networked computer services such as electronic mail and the Internet. By my signature below, I do release and indemnify, defend and hold harmless, Dover Public Library and the City of Dover from and against any and all claims, actions, suits, or proceedings of any kind or nature arising as a result of the actions of my child.  I also release the staff, City Council, City of Dover, and Dover Public Library without limitations, from damages, liabilities, costs, expenses, legal fees, and claims.
